Travel Medicine - Registered Yellow Fever Vaccination Centre. Travel vaccines are available at the surgery. It is advisable to see your doctor at least six weeks before travel. Vaccinations for travel purposes are charged to the patient and under no circumstances are covered by Medicare.

Women’s Health - pap smears, breast checks, antenatal care, family planning, pregnancy testing.

Children’s Health - vaccinations, child health and development assessments, behavioural and discipline problems, nightmares, bedwetting, acne, adolescent issues.

Health Checks – Recreational Scuba Diving ($95.00), Driving, Insurance, Pre-Employment, Pre-Travel, skin checks, yearly check ups, Lung Function (Spirometry), Audiometry (Hearing).

Lifestyle issues - weight control, quit smoking, drug problems, relationship issues and counselling, exercise programs, alcohol issues, nutrition.

General Health – acupuncture, asthma and allergies, blood pressure, care for the elderly, freezing (Liquid Nitrogen) of skin lesions, minor surgery (including skin lesions), diabetes management, relaxation and stress management, depression.

Work Cover Consultations - If your consultation is Work Cover related your employer will be billed directly for your treatment. You will be asked for your employer contact details so we may confirm that they will accept liability. You will be given 48 hours to submit your claim to your employer. If no formal arrangement regarding billing is in place between The Park Medical Centre and your employer, you will be required to pay for the consultation fees on the day.

Third party consultations are also available.

Repeat Prescriptions and Referrals
The policy of this practice is that repeat prescriptions for regular medications or referrals cannot be obtained without a consultation. It is preferred that you make an appointment for a consultation so your condition may be reviewed and the management of your condition updated.

Pathology and other Results
To protect your confidentiality and to maintain the highest quality of patient care, results will not be given over the telephone unless prior arrangement with the doctor is made. The doctors or nurse will advise you when your results should be available. You should make a follow up appointment to see your doctor. If your results are completely normal there will be no need to come in urgently. You will be able to discuss these results at your next consultation.

Transfer of Medical Records

Any records that require photocopying or posting via registered post or are requested by an insurance company or solicitor etc. will incur a fee.

For incoming records: Please collect a transfer form (pink) from the front desk, then ask the doctor to sign it. After you and the doctor have signed the transfer form take it/or send it to your previous doctor, and ask them to send us your records. If you are unable to see your previous doctor and wish us to call and arrange to transfer on your behalf, this can be organised, although you will still need to fill in the transfer form.  This will also attract a $33 (GST included) administration charge, in addition to any fees which your pre-exisitng doctor may charge for the release of your records.

For outgoing records: To have your records transferred from this practice to another, you will need to give us your written consent to release them.  Whether they are released by fax or post, we require that you pay $33 (GST included) record-release administration charge before we are able to release the records. If you wish to have us fax the records, we will need to receive a fax from the fax machine you want us to send your records to. If you wish to come in and pick them up, this can be arranged. We can send your records by post, however, this does attract a further fee of $5 to cover the cost of Registered Mail, which is the most secure method of posting medical records.

This practice will not accept a patient’s previous medical record until the patient has had a consultation and the treating doctor requests his/her history. As this practice has computerised medical records, we will only request and accept a summary from your previous doctor, not a complete paper record.

Home Visits
If you wish your doctor to make a home visit, please call the surgery to arrange a suitable time. Home visits are made at the doctors’ discretion and are only available for existing patients who live within 1km of the surgery.
